Försiktigt framåt för Obamacare i Kalifornien och Kentucky

Krisen kring president Obamas sjukförsäkringsreform fortsätter, och det är långt ifrån säkert att reformen kommer att överleva. Men trots att det har varit i det närmaste omöjligt för behövande att ansöka om sjukförsäkring på den federala sjukförsäkringssajten så finns det spirande tecken på att flera miljoner fattiga amerikaner verkligen kommer att ansöka om de bara ges en chans.

The Obama administration on Wednesday announced a one-year delay in a major element of the new health care law that would allow small businesses to buy insurance online for their employees through the new federal marketplace.

It was yet another setback for the rollout of the health care law and resulted, in part, from the well-documented problems of the insurance marketplace website. Administration officials said they had to focus on the basic functions of the website, so that individuals could shop for insurance, before offering online enrollment for small businesses. In the meantime, businesses and their employees can apply through brokers.

Many employees of small businesses are uninsured, and the businesses themselves are much less likely than big companies to provide coverage to workers and their families.

The state’s enrollment figures represent a rare bright spot in the unfolding story of the Affordable Care Act. Its rollout has been troubled by technical problems with the federal health care website, lower-than-expected enrollment and a public outcry over its role in the cancellation of millions of insurance plans.

Officials said 18- to 34-year-olds made up 22.5 percent of the nearly 31,000 Californians who selected a private health plan in October. The same age group makes up 21 percent of the state’s population.

The enrollment of young people is important to insurers because their relative good health offsets the costs for people with serious medical conditions.

“Enrollment in key demographics like the so-called young invincibles is very encouraging,” Peter V. Lee, the executive director of Covered California, said in a statement.

On the campaign trail, Senate Minority Leader Mitch McConnell was still blasting the new health-care law as unsalvageable. At the White House, President Obama was still apologizing for the botched federal Web site.

But in a state where the rollout has gone smoothly, and in a county that is one of the poorest and unhealthiest in the country, Courtney Lively has been busy signing people up: cashiers from the IGA grocery, clerks from the dollar store, workers from the lock factory, call-center agents, laid-off coal miners, Kentucky Fried Chicken cooks, Chinese green-card holders in town to teach Appalachian students.

In a state where 15 percent of the population, about 640,000 people, are uninsured, 56,422 have signed up for new health-care coverage, with 45,622 of them enrolled in Medicaid and the rest in private health plans, according to figures released by the governor’s office Friday.

If the health-care law is having a troubled rollout across the country, Kentucky — and Breathitt County in particular — shows what can happen in a place where things are working as the law’s supporters envisioned.

Det är med andra ord fortfarande möjligt för president Obama att finna fajten, även om marginalerna är mycket tunna.

Nästa års kongressval kommer med stor säkerhet till stor del att handla om Obamacare, och det är inte alls omöjligt att det blir det valet som till slut kommer att avgöra om reformen kan överleva eller inte.

    De guvernörer som styr de demokratiska stater där reformen rullats ut smidigt måste ju annars kunna sägas ha ett jättestarkt argument att komma med om de ställer upp i presidentvalet 2016.

    Obama vann 2008 och 2012 mycket på att han utlovade reformen om jag förstått rätt. Kentuckys och Kaliforniens guvernörer kommer att kunna gå till val 2016 på att de dessutom är kompetenta nog att ha klarat att genomföra den i sina egna stater!

    Och om de spelar kortet rätt så borde väl det faktum att reformen GÅR att genomföra i de stater där viljan finns att göra det själv utan att förlita sig på Washington också gynna demokraternas kandidater redan 2014 i övriga delstater? Ser framför mig guvernörskandidatdebatter med repliker av typen “the only thing standing between health care and the working poor in this state is our republican governor, elect me instead and I’ll give you what they already have even in Kentucky”.
